- WinDbg provider no longer picks arbitrary module[0] as name/base
(was showing "WS2_32" for kernel dumps). Name is now generic
"WinDbg (Live)" / "WinDbg (Dump)", base stays 0 so controller
doesn't override user's address.
- Added throttled read failure logging to WinDbg provider.
- New tabs (File→New Class, workspace right-click) inherit the
current tab's source/provider so users don't have to re-attach.
- Updated WinDbg provider tests for new behavior.
Type chooser:
- Fix PointerTarget mode hiding primitives due to stale modifier state
- Preselect */[n] modifier buttons to reflect current node type
- Primitive pointer support: int32*, double**, etc with provider deref
- hex64*/ptr64* with * modifier falls back to void* (meaningless deref)
- isValidPrimitivePtrTarget guard in controller, compose, format
- Modifier toggle no longer resets list selection
- Primitive pointers open FieldType mode (not PointerTarget)
- Type edit requires double-click (was single-click, too easy to misclick)

Self-referential struct children (e.g. Ball containing Ball) now render
as collapsed struct headers with a cycle marker. Clicking the fold margin
materializes the referenced children and auto-expands the recursive child,
allowing unlimited depth exploration.

RootClassName and RootClassType edit handlers now use m_viewRootId
instead of blindly picking the first root struct. Default name for
unnamed structs changed from <no name> to NoName. Dim the opening
brace on the command row to match the rest of the bar's grey text.

Collapse Provider interface from 9 virtual methods to 2 (read + size),
move providers to src/providers/, add name()/kind()/getSymbol() virtuals.
Replace FileProvider with BufferProvider, add ProcessProvider (Win32)
with module-based symbol resolution, wire ProcessPicker dialog, and
integrate getSymbol into pointer display and command row.
- Fix isReadable overflow for large addresses
- Guard deferred showSourcePicker/showTypeAutocomplete against stale edits
